About the Role

We are seeking a Chart Retrieval Specialist to support medical record collection and risk adjustment initiatives throughout New Jersey. This field-based role serves as the primary liaison between healthcare providers and the organization, ensuring timely and accurate retrieval of medical records from physician offices and healthcare facilities. The ideal candidate is comfortable working independently, traveling regularly, building relationships with physicians and office staff, and handling confidential medical information in accordance with HIPAA requirements. This position requires strong communication skills, professionalism, and the ability to manage multiple priorities while meeting productivity and quality standards.

Responsibilities

  • Travel to physician offices and healthcare facilities throughout New Jersey to obtain medical records.
  • Serve as the primary point of contact for assigned provider practices regarding chart retrieval activities.
  • Contact, schedule, and conduct provider office visits to request and collect medical records.
  • Educate physicians and office staff on chart retrieval and risk adjustment programs.
  • Build and maintain positive working relationships with healthcare providers and office personnel.
  • Retrieve medical records electronically and/or scan records onsite at provider locations.
  • Respond to provider inquiries and resolve issues that may impact record collection.
  • Follow up with physician offices to maximize chart retrieval success rates.
  • Review medical records to ensure completeness, accuracy, and compliance with established requirements.
  • Process chart requests and maintain accurate documentation within databases and tracking systems.
  • Meet established productivity, quality, and turnaround expectations.
  • Identify barriers, trends, or opportunities during provider interactions and communicate findings to leadership.
  • Maintain strict confidentiality of patient information and adhere to HIPAA regulations.
  • Manage multiple provider relationships, requests, deadlines, and projects simultaneously.

Requirements

  • Bachelor's degree or equivalent combination of healthcare experience and education.
  • Minimum of 2 years of healthcare industry experience.
  • Valid New Jersey driver's license with a satisfactory driving record.
  • Ability and willingness to travel extensively throughout New Jersey using a personal vehicle (mileage reimbursed at the current IRS rate).
  • Strong communication, interpersonal, and relationship-building skills.
  • Ability to work independently and effectively manage multiple priorities.
  • Proficiency with Microsoft Office applications.
  • Experience handling confidential medical information and maintaining HIPAA compliance.

Preferred Qualifications

  • Experience working directly with physicians, healthcare providers, or medical office staff.
  • Background in provider relations, medical records, chart retrieval, risk adjustment, HCC coding, healthcare outreach, or physician office operations.
  • Experience with EMR systems such as Epic, Allscripts, eClinicalWorks, or NextGen.
  • Familiarity with medical record review and healthcare documentation processes.

Pay

$35.00/hour
