About the role
The Association for Individual Development (AID) seeks a Case Manager Mental Health Professional (CM MHP) bilingual HOPWA, Elgin, IL. This position aligns with AID's mission to empower individuals with various challenges to live dignified and purposeful lives.
Responsibilities
- Manage individuals' housing including paperwork, lease maintenance, landlord relationships, and medical and behavioral health services.
- Obtain and maintain all necessary trainings to comply with TCHIP/HOPWA requirements.
- Participate in meetings and committees to foster good relationships with the HOPWA team and surrounding area teams.
- Document services accurately and in accordance with state and agency standards.
- Work collaboratively with a team of behavioral health service providers and external agencies.
- Meet scheduled hours and be flexible to meet client needs.
- Complete necessary paperwork, incident reports, and case notes.
- Facilitate communication and coordination with AID staff and community professionals.
- Maintain professionalism and adhere to agency policies and procedures.
- Meet productivity standards and complete required case management documentation.
- Assist clients with daily living activities, budgeting, and financial management.
- Monitor and report on the safety of clients' living arrangements.
- Acquire and maintain required certifications and trainings.
- Ensure proper maintenance of client records and record keeping.
- Provide effective services aligned with client needs and rights.
- Attend required clinical and team meetings.
Qualifications
- Bachelor’s degree in human services or a related field.
- Experience working in social services.
- No lifting restrictions.
- Ability to navigate stairs and provide services in clients' homes.
- Valid driver's license and liability insurance.
- IM+CANS certification.
