About the role
The Association for Individual Development (AID) is a non-profit organization dedicated to empowering individuals with diverse challenges to live dignified and purposeful lives. We seek a Case Manager Mental Health Professional or Qualified Mental Health Professional (MHP/QMHP) in Elgin, IL who shares our mission and is committed to making a difference.
Responsibilities
- Develop and implement individual treatment plans utilizing the IM+CANS to enhance independent community living skills.
- Complete documentation of mental health services in accordance with state and agency standards.
- Work collaboratively with a team of behavioral health service providers and external agencies to support a positive client-centered approach.
- Communicate and coordinate services with AID staff and professionals in the community via phone and email.
- Maintain professionalism and adhere to all policies and procedures.
- Facilitate client interactions and provide guidance to MHP and RSA staff.
- Monitor and report on the safety of clients' living arrangements.
- Manage client finances and obtain required certifications and trainings.
- Update client records and complete necessary paperwork accurately.
- Engage in client wellness initiatives and collaborate with healthcare professionals.
- Participate in team meetings and organizational events.
- Perform other related duties as assigned.
Qualifications
- Bachelor’s degree in Human Services or related field required for MHP; Master’s Degree in Human Services for QMHP.
- Experience working in social services required.
- No lifting restrictions; ability to navigate stairs and provide services in clients' homes.
- Valid driver's license and liability insurance as defined by AID’s Personnel Policy.
- Ability to drive a passenger vehicle and maintain IM+CANS certification.
