About the role
The Association for Individual Development (AID) seeks a bilingual Case Manager Mental Health Professional to support individuals with various challenges and promote their independence and dignity.
Responsibilities
- Develop and implement individual treatment plans using IM+CANS to enhance community living skills.
- Complete mental health service documentation in accordance with state and agency standards.
- Collaborate with a team of behavioral health providers to deliver a client-centered approach.
- Work scheduled hours and be flexible to meet client and program needs.
- Ensure compliance with agency, state, and federal regulations.
- Maintain professionalism and good boundaries in interactions with clients, coworkers, and external agencies.
- Meet productivity standards and complete required case management documentation promptly.
- Meet with clients regularly and document daily activities.
- Review and revise IM+CANS and update records as needed.
- Support clients with daily living tasks and provide training.
- Provide guidance to MHP and RSA staff on implementing IM+CANS goals.
- Acquire and maintain required trainings and certifications.
- Manage client finances and assist with bill payments, budgeting, and banking.
- Maintain accurate client records and complete necessary paperwork.
- Attend clinical supervision and team meetings.
- Recognize and respond appropriately to emergency situations.
Qualifications
- Bachelor’s degree in Human Services or Master’s degree preferred.
- Experience working in social services required.
- Fluent verbal and written Spanish and English.
- No lifting restrictions.
- Ability to navigate stairs and provide services in clients' homes.
- Valid driver's license and liability insurance.
- First Aid, CPR, and Non-violent Crisis Intervention training.
- Other relevant trainings as assigned.
