What You Will Do To Change Lives

To change lives under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders. The role involves assisting these individuals to live as independently as possible in their chosen community.

What You Bring To The Table (Must Have)

  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
  • Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.

San Diego JIMH ACT

This Assertive Community Treatment (ACT) Full Service Partnership (FSP) Team with Supported Housing and a Strength-Based Case Management (SBCM) component will serve 110 adults, ages 18 and over.
