Case Manager (Child & Adolescent Clubhouse Program)

Job Duties And Responsibilities

• Aid youth with service needs, develop supportive networks, and coordinate rehabilitation services.
• Collaborate with the individual and CORE services provider to create the individual service plan.
• Support the transition between levels of care and link with other agencies for needed resources.
• Assist in crisis situations, provide assessments, and monitor individuals.
• Establish professional relationships with youth, families, and coworkers.
• Provide direct service, complete necessary paperwork, and ensure eligibility criteria are met.
• Manage caseload by ensuring services are authorized, chart paperwork is complete, and consumer charts are closed.
• Document attempts to contact individuals, review ISP, complete monthly caseload reports, and track appointments and case management.
