Services include housing and emergency shelters, hunger and nutrition services, and other community support programs. VOA integrates compassion with highly effective programs that build and strengthen communities.

This Case Manager role is directly responsible for service provision and housing assistance for participants in programs at the Bannock Youth and Family Center. Current programs serve youth and youth-headed households ages 18-24 who are literally homeless, as well as participants with justice involvement. Service modalities include Motivational Interviewing and Critical Time Intervention.

Responsibilities

  • Ensure the participant’s voice is heard and their ideas play a prominent role in programming.
  • Possess in-depth knowledge of program requirements, including referral processes, support services, allowable costs, field-relevant best practices, coordinated entry systems, and HUD transitional and rapid rehousing standards.
  • Provide intensive case management and ensure regular participant engagement in services.
  • Collect necessary documents to obtain and maintain housing, identification, income changes, and household changes.
  • Identify and serve the unique needs of participant households using screening and assessment tools.
  • Develop specialized knowledge of service delivery database systems within the organization.
  • Attend intakes, housing orientations, lease-ups, and ongoing educational opportunities for participants.
  • Ensure timely reporting within data entry systems and hard copy files in accordance with grant guidelines.
  • Assist with community-wide outreach to identify eligible participant households facing substantial barriers to housing stability.
  • Utilize the OneHome system as required by funding sources.
  • Assist clients in housing searches, rental applications, lease interpretation, and understanding tenant rights and responsibilities.
  • Facilitate delivery of appropriate services for participant households as indicated by relevant service modalities.
  • Provide housing navigation and process referrals for participating households.
  • Conduct needs-based assessments in the community as needed.
  • Coordinate with the team to deliver services tailored to the needs of households.
  • Assist in relocating youth and securing emergency housing.
  • Attend orientations, trainings, education programs, staff meetings, community meetings, conferences, and workshops.
  • Document all client interactions along with required eligibility and demographic information.
  • Obtain and maintain literal homelessness documentation.
  • Maintain confidentiality and participate effectively within and across teams.
  • Facilitate the referral, intake, and orientation process for new YTP households.
  • Ensure housing for all participating households complies with program standards.
  • Complete housing quality inspections, rent reasonableness, and fair market rent (FMR) assessments.
  • Perform job responsibilities in accordance with the Social Work Code of Ethics.
  • Use a personal vehicle for securing and maintaining housing for enrolled participants.

Requirements

  • Bachelor’s degree in human services, social work, or a closely related field, or equivalent experience.
  • At least one year of direct, full-time case management experience working with youth experiencing homelessness.
  • Experience working with literally homeless populations, including youth and youth-headed households.
  • Familiarity with current housing market information, trends, and available resources.
  • Crisis intervention skills.
  • Colorado driver’s license and state-mandated automobile insurance.

Preferred Qualifications

  • One year of experience directly relevant to the program.
  • In-depth knowledge and skill in Critical Time Intervention and Motivational Interviewing.
  • Proficient written, oral, and interpersonal communication skills.
  • High competency in Microsoft Office Suite and cloud-based applications.
  • Strong time-management and prioritization skills.
  • Experience working both independently and in a team-oriented, collaborative environment.
  • Strong organizational skills.

Working Conditions

  • Must be willing to use a personal vehicle for service provision throughout a seven-county area.
  • Works in close proximity to other staff at the Bannock Youth & Family Center.
  • Full-time, 40 hours per week, Monday through Friday. Some evenings and weekends are required.
