About the Role
The LISA Division of Community Solutions is seeking an energetic and dedicated Case Manager to work at the SAIL Program with young adults, ages 18-23 years old. Case Managers will work closely as a team within the program and regularly collaborate with the Program Director, Youth Residential Supervisor, and Vocational/Life Skills Manager.
You will ensure that the tenets of strengths-based, relational, trauma-informed, gender-responsive, and culturally competent approaches are maintained within the program and services offered.
Responsibilities
- Support the development of an emotionally and physically safe environment for staff and residents.
- Maintain a caseload and facilitate duties related to intake, screening, assessment, orientation, service and discharge planning, and facilitation of services for assigned residents, including:
- Manage referrals and intakes with the Program Director/Youth Residential Supervisor.
- Aid in the selection of appropriate assessments and screening tools.
- Facilitate individual screens/assessments and develop individualized service and discharge plans collaboratively with residents.
- Actively collaborate with families, schools, and all providers serving residents.
- Initiate regular, individual case management sessions with residents.
- Collaborate with staff to ensure that each resident’s individual goals are attended to, particularly related to life skills-building.
- Contribute to facilitation of the program’s behavior motivation system (TIER).
- Brokers services in the community as needed.
- Aid in the selection of group offerings, plan regular groups/services, and facilitate groups.
- Assist in the planning/facilitation of service team meetings and actively participate in other meetings with families, DCF, and other providers.
- Provide quality supervision, coaching, and support to subordinates.
- Complete logs, reports, and data entry as required/requested.
- Maintain the confidentiality of the residents and their families according to the agency's policy.
- Provide appropriate role modeling for staff and residents at all times, including dress, managing personal emotions, and behavior.
- Collaborate with the Program Director/Youth Residential Supervisor to provide guidance and direction to program staff and to identify any gaps in services and areas of improvement.
Requirements
- Bachelor’s degree required, plus a minimum of 2 years’ experience in a similar capacity. Preference given to those with advanced degrees in Education, Social Work, Psychology, and other related fields.
- Requisite understanding and commitment to the principles of strengths-based, relational, trauma-informed, gender-responsive, and culturally competent approaches in the delivery of programming and services for abused/neglected and adjudicated youth.
- Excellent oral, written, and interpersonal skills.
- Experience with Microsoft Office.
- COVID-19 vaccination or a negative test prior to start date.
Schedule
Monday, Wednesday, Thursday, Friday: 8:00 AM to 4:00 PM
Tuesday: 10:00 AM to 6:00 PM
Pay
Starting wage: $23.84/hour
