Case Manager Youth and Family Team
About the role
Be part of the solution to the behavioral health crisis. As a team member for ND Department of Health & Human Services, you will ensure the most vulnerable have access to a high-quality healthcare system in which prevention works, treatment is effective, and people recover.
Responsibilities
- Provide individuals opportunities to manage their own recovery so they can live and work independently in their community.
- Conduct comprehensive needs assessments, complete, and implement service plans.
- Leverage contacts and referral partners to ensure access to needed services.
- Aid in crisis response.
Requirements
- A bachelor’s OR a master’s degree
- OR at least five years of paid supervised experience working with individuals with SMI/SED in a role with case management functions such as individual assessment, care plan development and maintenance, referral and appointment scheduling, monitoring and follow-up activities.
Qualifications
- Knowledgeable about resources offered for youth and families in your community
- Preferred to work with youth in their home and natural environments
Skills
- Valid driver’s license
- Successful completion of the assigned defensive driving course during new-hire orientation
