Description Summary

The Youth Case Manager provides case management services to children, adolescents, and their families to support healthy development, stability, and success at home, school, and in the community. The Youth Case Manager works collaboratively with youth, caregivers, schools, and community partners to identify strengths, address barriers, connect families to resources, and support achievement of individualized goals.

A Day in the Life

  • Conduct ongoing assessments of youth and family needs, strengths, and goals.
  • Develop and monitor individualized service plans with youth and families.
  • Connect youth and families to community resources, educational supports, recreational opportunities, healthcare services, and social service programs.
  • Advocate for youth and families to ensure access to appropriate services and supports.
  • Support youth in developing life skills, problem-solving abilities, self-advocacy, and positive coping strategies.
  • Collaborate with schools, service providers, caregivers, and community partners to coordinate care and services.
  • Provide crisis support and connect families with appropriate resources as needed.
  • Maintain progress toward goals and adjust service plans when necessary.
  • Document services, contacts, and progress accurately and timely.
  • Maintain productivity standards and service requirements established by the agency.
  • Participate in team meetings, supervision, and case reviews.
  • Complete required surveys, reports, and incident documentation as appropriate.

Requirements

  • Experience working with youth, families, schools, social services, behavioral health programs, community organizations, or related settings preferred.
  • High school diploma or equivalent required.
  • Associate degree, bachelor's degree, or relevant experience in human services, education, social services, behavioral health, childcare, youth development, or a related field is preferred but not required.
  • Candidates with lived experience, community-based experience, youth mentoring experience, or demonstrated ability to work effectively with children, adolescents, and families are encouraged to apply.
  • Valid driver's license, reliable transportation, and ability to travel within the community may be required.
