About the role
The Guidance Center is a recognized leader in the field, offering a wide array of services to over 14,200 individuals annually. As a Case Manager, you will play a crucial role in supporting and empowering those in need.
Responsibilities
- Conduct case management assessments to identify areas of need and service preferences
- Gather and integrate information from various sources to support assessment processes
- Utilize motivational interviewing techniques to assess and articulate consumer motivation
- Aid consumers in accessing entitlements, community services, and supports
- Provide complete and accurate information about available services and resources
- Facilitate the development of Individualized Plans of Service with consumers and treatment teams
- Engage in regular face-to-face and home visits to monitor and coordinate service delivery
- Coordinate with primary and other healthcare providers to ensure continuity of care
- Support crisis intervention and discharge planning, including community supports after hospitalization
- Assist with the transition process from inpatient to community settings
- Develop and maintain relationships with local providers and community organizations
- Document all service activities and maintain appropriate professional ethics and boundaries
Requirements
- Bachelor's degree in a human services field
- At least one year of experience treating or working with individuals diagnosed with mental illness
- Michigan Chauffeur License with a good driving record
- Ability to engage adult mental health consumers, their families, and supporters
- Expertise in helping consumers identify and build on their strengths, resources, and supports
- Nonjudgmental approach reflecting cultural competency and sensitivity
- Effective organizational, verbal, and written communication skills
- Ability to handle crisis situations appropriately

- Qualified Mental Health Professional credential within 120 days of hire
