About the role
This is a hybrid position based in the FIVCO region.
Responsibilities
- Determine the nature of referrals and assess the need for additional contacts or screenings.
- Provide intake into the IDDS system.
- Conduct home visits to identify family/individual priorities, resources, and concerns.
- Facilitate, develop, review, and monitor person-centered plans on an ongoing basis.
- Cookordinate, monitor, and ensure timely delivery of services outlined in the person-centered plan.
- Maintain thorough documentation and comply with all agency policies.
- Perform other duties as assigned in the current job description.
Requirements
- Minimum of a Bachelor’s degree in a Human Services field.
- Prior experience working with the IDD population is required.
Skills & Knowledge
- Strong working knowledge of community resources and how to access them.
- Ability to engage effectively with individuals and families regarding developmental disability concerns.
- Proficiency in computer systems and electronic documentation.
