Care Manager - Mary J. Blige Center

Responsibilities

• Enrolling individuals and completing the intake and documentation.
• Developing individualized, person-centered action plans to address clients’ specific needs.
• Connecting individuals to essential community resources, and conducting follows-ups as needed.
• Processing and maintaining Social Care Network billing, as well as other billing and reimbursement responsibilities as assigned.
• Meting regularly with participants to monitor/document progress and assist in addressing barriers and goal progression.
• Building and maintaining relationships with community organizations to facilitate resources access.
• Other duties as assigned.

Requirements

• Bachelor's degree in Social Work or a related field.
• Familiarity with community resources and case management practices for high-risk populations.
• Bi-lingual Spanish proficiency preferred.
• Strong interpersonal, engagement, and communication skills.

Pay

The salary range is $50,000-$52,000.
