Care Integration Coordinator Full- time Day
About the role
The Care Integration Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, accountabilities include coordinating communication between patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services provided and regulatory compliance.
Responsibilities
- Coordinate communication between patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care.
- Support the delivery of quality patient care by ensuring the maintenance of accurate records, process documentation and supporting the clinical team.
- Handle multiple tasks simultaneously, requiring excellent organizational skills and attention to detail.
Requirements
- 2 years of administrative experience, healthcare setting preferred or demonstrated knowledge of healthcare terminology.
Qualifications
- Demonstrated knowledge of healthcare terminology.
- Associate's Degree in Business, Healthcare or related field preferred.
Skills
- Excellent organizational skills.
- Attention to detail.
- Ability to handle multiple tasks simultaneously.

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $24.00 - $35.77 per hour.
Schedule
This position is Full-time and will work 8-hour, Day shifts.
