About the role
The Care Integration Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, this role coordinates communication between patients, healthcare providers, insurance companies, and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services, and regulatory compliance. The goal is to maintain accurate records, process documentation, and support the delivery of quality patient care.
This role requires excellent organizational skills, attention to detail, and the ability to handle multiple tasks simultaneously.
Responsibilities
- Coordinate communication between patients, healthcare providers, insurance companies, and other stakeholders.
- Ensure safe and efficient transitions of care.
- Maintain accurate records and process documentation.
- Support the delivery of quality patient care.
- Ensure payment for clinical services provided and regulatory compliance.
Requirements
- 2 years of administrative experience, preferably in a healthcare setting, or demonstrated knowledge of healthcare terminology.
Qualifications
- Associate's Degree in Business, Healthcare, or a related field (preferred).

Pay
The pay range for this position is $25.00 - $35.77 per hour. These amounts are the base pay range; additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
Schedule
- Full-time, 8-hour day shifts.
