About the role

The Care Facilitator at St. Mary's Medical Center is a full-time position within the Care Management Department. This role involves exercising independent judgment and professional discretion in case management, collaborating with physicians and residents, advocating for patients and families, monitoring resource utilization, and preparing patients for discharge.

Responsibilities

• Collaborate with attending physicians and residents to support patients through their acute episodes of care.
• Act as an advocate to help patients and families navigate the healthcare system.
• Maintain oversight of patients' continuing stays to ensure appropriate resource utilization.
• Prepare patients for a safe and timely discharge by addressing their needs and those of the medical team.
• Use collaborative problem-solving skills to address challenges and improve patient outcomes.

Requirements

• Bachelor's degree in a related field (e.g., nursing, social work, public health).
• Minimum 2 years of experience in case management or related healthcare field.
• Current certification in case management or relevant healthcare credential.
• Strong communication and interpersonal skills.
• Proficiency in Microsoft Office Suite.
