About the role

The Care Facilitator at St. Mary's Medical Center is a full-time position within the Care Management Department. This role involves exercising independent judgment and professional discretion in case management, collaborating with physicians and residents, advocating for patients and families during acute episodes of care, monitoring resource utilization, and facilitating safe and timely patient discharges.

Responsibilities

• Collaborate with attending physicians and residents to manage patient cases
• Advocate for patients and families during acute episodes of care
• Maintain oversight of patients' continuing stays to ensure appropriate resource utilization
• Facilitate patient transitions to home or other settings in a manner that meets their needs and those of the healthcare team
• Use collaborative problem-solving skills to address challenges and improve patient outcomes

Requirements

• Bachelor's degree in a related field (e.g., nursing, social work, public health)
• Minimum 2 years of experience in case management or related healthcare field
• Current certification in case management (if applicable)
• Strong communication and interpersonal skills
• Proficiency in Microsoft Office Suite
