About the role

The Care Facilitator at St. Mary's Medical Center will provide independent case management services to patients within the Care Management Department. This role involves collaborating with physicians and residents, advocating for patients and families during acute episodes of care, monitoring resource utilization, and facilitating safe and timely patient discharges.

Responsibilities

• Collaborate with attending physicians and residents to support patients through their acute episodes of care.
• Advocate for patients and families to ensure they have the necessary resources and information to navigate their healthcare journey.
• Maintain oversight of patients' continuing care to monitor resource utilization and identify potential issues early.
• Work proactively to resolve problems and improve patient outcomes, ensuring that patients are prepared for a safe and timely discharge from the hospital.

Requirements

• Bachelor's degree in a related field (e.g., nursing, social work, public health).
• At least 2 years of experience in case management or a related healthcare field.
• Knowledge of healthcare policies and procedures.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Qualifications

• Current certification in case management or a related field (e.g., Certified Case Manager, Certified Rehabilitation Counselor).
• Experience working with diverse populations and cultural sensitivity.
• Proficiency in Microsoft Office Suite.
