About the role

The Casual Care Facilitator will provide case management services to patients within the Care Management Department at St. Mary's Medical Center. This role involves exercising independent judgment and professional discretion in managing patient cases, collaborating with healthcare providers, and advocating for patients and their families during the acute episode of care.

Responsibilities

• Collaborate with attending physicians and residents to ensure effective patient care coordination
• Serve as an advocate to help patients and families navigate the acute episode of care
• Maintain oversight of patients' continuing stays to monitor resource utilization
• Prepare patients for a safe and timely discharge
• Use collaborative problem-solving skills to address the needs of patients and healthcare providers

Requirements

• Bachelor's degree in a related field (e.g., healthcare, social work)
• Minimum 2 years of experience in case management or related field
• Current certification in case management (if applicable)
• Strong communication and interpersonal skills
• Proficiency in Microsoft Office Suite
