About the role
The Veterans Care Coordinator/Navigator provides care coordination/patient navigation services to Jefferson County veterans engaged in behavioral health services at the Jefferson County Community Mental Health Program (CMHP) - CCBHC/Certified Community Behavioral Health Clinic.
Responsibilities
- Direct interactions with veterans in CMHP-CCBHC services, offering support and addressing needs.
- Make telephone contacts to assist veterans in crisis, arrange for mental health and substance abuse services, and remind clients of upcoming appointments.
- Transport clients to necessary counseling/treatment appointments when appropriate and with supervisor approval.
- Help veterans connect with medical services and refer them to OHP Assisters if needed.
- Assist with referrals to various services such as food banks, LINC programs, and community college.
- Provide case management services, including documentation, and help clients develop and connect to pro-social or wellness-oriented recovery activities.
- Collaborate with the CCBHC-CMHP behavioral health treatment team and Prevention-Health Education Team on suicide prevention strategies and community trainings.
- Perform other duties as assigned by the Program Director or Mental Health-Adult Team Program Manager.
Requirements
- Education and/or Experience: High School diploma or equivalent; bachelor’s degree or 5 years of relevant experience in the behavioral health field.
- Familiarity with veterans/services and mental illness/substance abuse populations.
- If in recovery, must demonstrate continuous sobriety for the past two years.
- Licenses and Certifications: CADC-I or QMHA-I certification required or eligibility to register within 1 week of hiring to obtain QMHA-I within 6 months or CADC-I within 1 year. Must maintain a valid Oregon Driver License or equivalent.
- Preferred: Associate’s degree in a behavioral health field; bilingual in English/Spanish.
