Care Coordinator

About the role

The Care Coordinator supports children with developmental, physical, or severe emotional disabilities through the Children's Long Term Support (CLTS) Waiver Program. They serve as the primary contact for families, assisting with service access, assessment, planning, coordination, and ongoing support.

Responsibilities

• Conduct functional assessments and reassessments using approved tools
• Develop individualized service plans with families and providers
• Authorize, coordinate, and monitor services in line with plans
• Facilitate family-centered team meetings and maintain accurate documentation
• Collaborate with service teams and supervisors to determine appropriate interventions
• Testify at legal proceedings when necessary and participate in staff training

Service Delivery

Promote client independence and growth through trauma-informed, family-centered services, utilizing evidence-based practices. Ensure accurate time reporting and documentation.

Qualifications

• Bachelor's degree in a health or human services field (e.g., Social Work, Psychology, Counseling, Special Education, Occupational Therapy, Sociology, Criminal Justice)
• At least 1 year of experience working with children in the target population
• Strong communication, organizational, and documentation skills
• Proficiency with electronic health records and computer applications
• Valid driver’s license, reliable transportation, and satisfactory driving record
• Bilingual fluency in Spanish is preferred but not required
