Care Coordinator

About the role

The Care Coordinator supports children aged 0–3 with developmental delays or disabilities and their families through a family-centered, trauma-informed approach within an Early Intervention program. This is a community-based role involving travel, physical activity, and exposure to various environments.

Responsibilities

  • Conduct assessments
  • Develop and implement individualized service plans
  • Coordinate services
  • Facilitate family-centered team meetings
  • Support families via education, advocacy, and collaboration
  • Maintain accurate documentation
  • Collaborate with internal teams and community providers

Qualifications

  • Bachelor's degree in Social Work, Child Development, or related field
  • One year of supervised experience with families (preferred)
  • Valid driver's license and reliable transportation
  • Strong communication skills
  • Organizational skills
  • Ability to work in diverse settings
  • Ability to respond appropriately in crisis situations (essential)
