About the role
The Care Coordinator supports children aged 0–3 with developmental delays or disabilities and their families through a family-centered, trauma-informed approach within an Early Intervention program. This is a community-based role involving travel, physical activity, and exposure to various environments.
Responsibilities
- Conduct assessments
- Develop and implement individualized service plans
- Coordinate services
- Facilitate family-centered team meetings
- Support families via education, advocacy, and collaboration
- Maintain accurate documentation
- Collaborate with internal teams and community providers
Qualifications
- Bachelor's degree in Social Work, Child Development, or related field
- One year of supervised experience with families (preferred)
- Valid driver's license and reliable transportation
- Strong communication skills
- Organizational skills
- Ability to work in diverse settings
- Ability to respond appropriately in crisis situations (essential)
