About the Role

The Care Coordinator is a full-time, on-site role based in Las Vegas, NV, responsible for organizing and overseeing home care services for clients. This role involves assessing client needs, developing care plans, and coordinating schedules and services between clients, caregivers, and other care professionals. The Care Coordinator maintains regular communication with clients and families, monitors the quality of care, and responds promptly to concerns, changes in condition, or service requests.

Responsibilities

  • Assess client needs and develop personalized care plans.
  • Coordinate schedules and services between clients, caregivers, and other care professionals.
  • Maintain regular communication with clients and families to monitor care quality.
  • Respond promptly to concerns, changes in condition, or service requests.
  • Update records and ensure compliance with company policies and state regulations.
  • Support caregivers with guidance and resources.
  • Collaborate with the office team to improve processes and promote client satisfaction.
  • Uphold Assisting Hands® Home Care’s standards of excellence.

Qualifications

  • Demonstrated skills in Care Coordination and Case Management to assess needs, develop care plans, and manage ongoing services.
  • Strong Communication and Customer Service skills to work effectively with clients, families, caregivers, and internal teams.
  • Understanding of Home Care practices and regulations, with the ability to support caregivers and uphold quality standards.
  • Organizational and time-management skills to handle multiple cases, schedules, and documentation accurately.
  • Experience in healthcare, social services, or home care coordination preferred.
  • Proficiency with basic computer applications and care management software or databases.
  • High school diploma or equivalent required; additional education in healthcare, social work, or related fields is a plus.
  • Ability to work on-site in Las Vegas, NV, and occasionally travel locally for client visits, as needed.
