Care Coordinator Plus - Amsterdam

About the role

Join us in our mission as a Health Home Care Coordinator Plus at Catholic Charities of Care Coordination Services. You will play a pivotal role in individuals' lives, making a significant impact through:

Responsibilities

• Accept referrals from supervisor and attempt to make contact with referrals within program guideline timeframes
• Gather input from providers to complete a comprehensive assessment
• Advocate with appropriate agencies for services and assist members in the scheduling and coordination of services
• Conduct home visits, as appropriate, and monitor member health status, access to services and quality of received services
• Document updates to care plan and assessment, including barriers and service access
• Research and obtain current information on targeted chronic illnesses

Qualifications

• Minimum: Bachelor's Degree in child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreation therapy, rehabilitation, social work, sociology, or speech and hearing; OR NYS licensure and current registration as a Registered Nurse and a bachelor’s degree; OR A Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; OR A Credentialed Alcoholism and Substance Abuse Counselor (CASAC)
• Preferred: Licensed level clinician with prior experience in a behavioral health clinician or care management supervisory capacity; Working knowledge of the NYS Health Home Program.
• Experience: Either providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse; OR Linking persons who have been diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse to abroad range of services essential to successfully living in a community setting

Pay

Minimum Salary: $23.07
Maximum Salary: $23.07

Schedule

Monday - Friday 8:30-4:30

Prefered

• Licensed level clinician with prior experience in a behavioral health clinician or care management supervisory capacity
• Working knowledge of the NYS Health Home Program.
