Care Coordinator, Pharmacy Population Health

About the role

The Care Coordinator position at Sentara is a hybrid role requiring 2-3 days per week in the office at Sentara Corporate Facilities, with additional days on-site at affiliate practices. This position allows for some work from home days.

Responsibilities

• Develops, participates, and monitors multidisciplinary collaboration of services to patients where appropriate.
• Assists adult patients and their families with personal and environmental difficulties associated with medical conditions up to and including terminal illnesses.
• Performs a variety of casework duties and provides case management services to patients.

Requirements

• Bachelor's Level Degree (Required)
• 1-year of Long-Term Care experience (Required)
• Ambulatory and Clinical Pharmacy experience (Preferred)
• 1-Year Medicaid or Health Plan experience (Preferred)

Qualifications

• Education: Ideal candidate should ideally have a Certified Medical Assistant (CCMA) or LPN license.
• Certification/Licensure: Virginia Pharmacy Tech License and CPhT License (Received through the NHA or PTCB) OR Certified Medical Assistant (CMA), or Registered Medical Assistant (RMA), or Certified Clinical Medical Assistant (CCMA) OR LPN Virginia state license.

Skills

• Strong communication and interpersonal skills.
• Ability to manage multiple tasks and prioritize effectively.
• Knowledge of healthcare policies and procedures.
