Job Summary
The Care Coordination Supervisor is responsible for assessing patients' clinical needs and ensuring accurate placement within Berkshire Health Systems. This role integrates care management skills with clinical expertise to make informed placement decisions, supporting both patient care and the organization’s strategic goals. The Care Coordination Supervisor serves as the clinical expert and decision-maker for daily patient throughput and placement decisions, guiding the placement of patients to appropriate care settings. Job Requirements
- Experience: Five years of clinical nursing experience.
- Education: Bachelor's Degree BSN
- Licensure & Certifications: RN - MA RN License
