BA Social Worker - SGH Outpatient Behavioral Health - Day Shift - Full Time

AWS Hours Requirement: 8/40 - 8 Hour Shift

Pay

The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade will be dependent on factors such as years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, and employer business practices.

• Hourly Pay Range: $32.730 - $40.910 - $45.810

Responsibilities

• Provide documentation for discharge planning, psychosocial assessments, treatment plans, UR review, and care coordination for patients.
• Handle intake phone calls and assess patients for appropriate programs.
• Document all discharge planning and progress towards goals.
• Consult with team members in the development of discharge plans.
• Maintain ongoing communication with multidisciplinary team members regarding patient status and follow-up.
• Attend rounds in major practice areas.
• Document consultation, recommendations, interventions, and treatment plans.
• Communicate needs for patient care conferences.
• Refer to internal hospital resources that can assist patients and families.
• Refer basic community resources and assist with discharge planning for patients and families.
• Provide referrals for ongoing assistance to patients and families.
• Consult and intervene in crisis situations, assisting staff in de-escalating volatile situations.
• Participate in critical incident debriefings.
• Assist clinical staff with locating community resources.
• Assist with referrals to patient's family for ongoing assistance after leaving the program.
• Facilitate a timely discharge of patients.
• Report all mandatory protective concerns such as Elder Abuse, Child Abuse, and Domestic Violence as required by law.
• Protect patient privacy by maintaining confidentiality and keeping current with regulatory and legal requirements.
• Identify bio-ethical dilemmas and initiate appropriate responses.
• Assess discharge plans, including completing psychosocial assessments as appropriate per scope of practice and program.
• Identify key clinical issues such as grief and loss, adjustment to illness, and psychiatric concerns with attention to developmental milestones and life transitions.
• Practice appropriate boundary setting with clients and demonstrate sensitivity to cultural and ethnic diversity.
• Perform effective crisis intervention and problem-solving techniques, serving as a resource to others in these areas.
• Seek resources when faced with unusual or new situations.
• Evaluate patient/family response to interventions and revise care provided accordingly.
• Orient patients, initiate, and support the admissions process per policy and department guidelines.
• Complete Utilization Review for admitting and ongoing treatment for patients.
• Document authorizations in the electronic health record per policy or department guidelines.
• Adequately orient new employees on rules and responsibilities and evaluate their abilities and efficiency.

Requirements

• Bachelor's Degree in Social Work or other behavioral health-related field.
• AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association (REQUIRED).

Preferred Qualifications

• Hospital experience.
• Case management experience.

Skills

• Good customer service skills and experience working with a diverse population.
