Associate Patient Care Coordinator

Primary Responsibilities

Taking incoming calls, from call center and patients
Providing excellent customer service
Managing day-to-day operations for activities and programs
Answering phones and scheduling appointments as necessary, verifying correct visit type and primary care provider
Answering telephone calls, re-directing calls as appropriate, assisting callers with questions or concerns, and taking messages and creating and managing tasks as needed
Managing medical records (maintaining, filing/scanning, preparing for schedule)
Ensuring all correspondence is scanned and/or filed, processing requests for medical records release and maintaining appropriate logs, etc.
Working with back-office staff to ensure smooth patient flow
Resolving service issues and complaints, and deferring to appropriate management or clinical personnel when necessary
Closing out EMR messages
Establishing and maintaining effective working relationships with patients, employees, and the public
Performing all other related duties as assigned

Qualifications

• High School Diploma/GED (or higher)
• 1+ years of experience in customer service
• Basic level of proficiency with Microsoft Office

Preferred Qualifications

• 1+ years of work experience with medical office processes
• 1+ years of related work experience including data entry
• Prior experience with EMR computer applications
• Prior experience with medical software
• Knowledge of working through medical portal systems
• Knowledge of medications and medical terminology
