Associate Patient Care Coordinator

Primary Responsibilities

  • Call patients to remind them of scheduled appointments
  • Support the Clinical team in chart preparation and request medical information
  • Explain the process of the wellness visit to patients including, but not limited to what is involved, who the provider is, and the location of the specialist office
  • Responsible for obtaining "expert" level knowledge of the inter-workings of the Medicare Advantage plans
  • Responds effectively to incoming patient calls and communicates pertinent information to designee to provide for continuity and optimal care
  • Attends departmental meetings and provides constructive recommendations for areas of improvement
  • Documents pertinent patient information in the electronic medical record as it relates to job function
  • Covers with various providers, team members and outreach team regarding any scheduling guidelines
  • Maintains established productivity, schedule adherence, and quality standards
  • Uses, protects, and discloses Optum patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
  • Performs additional duties as assigned

Qualifications

  • High School Diploma/GED
  • 1+ year of medical front office experience
  • Intermediate level of computer proficiency (multi-task through multiple applications including Microsoft Outlook, Excel, and Word)
  • 1+ year of related healthcare experience, including scheduling experience
  • Knowledge of medical terminology
  • EMR experience
  • Office Administrative / Receptionist / Clerical experience in a medical environment

  • Excellent communication skills and phone etiquette
  • Strong attention to detail
  • Ability to multi-task
  • Ability to manage time effectively and work independently

Pay and Schedule

The hourly pay for this role will range from $16.00 to $29.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
