Aging Care Management Supervisor 2 (Local Government) - Lebanon County

Responsibilities

• Protective Oversight: Oversee investigations, evaluations, and maintenance of protective service cases in compliance with laws and regulations.
• Staff Guidance: Train workers on protective service rules and procedures and review performance to support improvement.
• Case Review: Monitor ongoing protective service cases and confirm accuracy of Reports of Need for adults of all ages.
• Community Coordination: Work with nursing homes, personal care homes, hospitals, law enforcement, and other entities to share information and support consumers.
• Program Administration: Update protective service plans, monitor programs with the Pennsylvania Department of Aging, and implement corrective actions as needed.
• Scheduling Support: Prepare on-call schedules, ensure coverage, and assist workers with on-call needs during non-working hours.

Schedule

Full-time employment. Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch. This position does not offer telework options.

Requirements

• Four years of experience in public or private social work; and a Bachelor's Degree; or
• Any equivalent combination of experience and training, including successful completion of 12 semester hours of college-level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
• Must meet Pennsylvania residency requirements.
• Must be able to perform essential job functions.
• A conditional offer of employment will require submission of criminal history reports.
