Aging Care Manager 2 (Local Government) - Wayne County Area Agency on Aging

About the role

The Wayne County Area Agency on Aging seeks a compassionate, organized individual to help older adults access services and supports that allow them to remain active, safe, and independent in the community.

Responsibilities

• Care Assessment: Complete intake and reassessment duties and document information in the appropriate systems
• Service Coordination: Arrange needed services and work with community partners to support consumer needs
• Program Guidance: Explain program eligibility, limits, and procedures to consumers and families
• Case Maintenance: Manage caseload tasks such as reports, records, and required follow-up contact
• Information Review: Interpret requests from professionals and determine appropriate referrals

Qualifications

• Minimum Experience and Training Requirements: Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
• Other Requirements: Must meet the PA residency requirement.
• Legal Requirements: Must pass a background investigation. A conditional offer of employment will require a drug screening. This position falls under the provisions of the Child Protective Services Law. A conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
