Aging Care Manager 2 (Local Government) - Lehigh County Area Agency on Aging

About the role

You will develop care plans and manage services for older adults to help them have access to the programs, support, and information they need to remain active, healthy, productive, and as independent as possible in their communities. Use your professional experience, people skills, and problem solving abilities to uplift and advocate for older adults and their families.

Responsibilities

• Assess needs, develop care plans, and provide and coordinate services for older adults
• Support consumers with social, economic, emotional, health, and physical problems and their families and caregivers
• Prepare case histories and social assessment forms; obtain medical assessments and financial information to determine eligibility and develop a plan of care
• Maintain accurate records of past and ongoing care; prepare memos, letters, summaries, and reports
• Help consumers manage incomes, access long-term services and supports, and provide information to consumers and their families about available options
• Work closely with other social service agencies, professional staff, hospitals, clinics, courts, and community resources to coordinate care
• Provide comprehensive support and services, including home visits, to people living with dementia and their caregivers under the Guiding an Improved Dementia Experience (GUIDE) Model

Requirements

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training

You must meet the PA residency requirement.

This position falls under the provisions of the Older Adult Protective Services Act. A conditional offer of employment will require submission and approval of satisfactory criminal history reports, including PA State Police and FBI clearance.
