Admissions and Referral Intake Specialist
About the role
The Admissions & Referral Intake Specialist supports admissions to the facility, in-house transfers and discharge processing for the Skilled Nursing, TCU and Assisted Living programs. Works in a team-based setting to work collaboratively with hospitals, skilled nursing facilities, referral agencies, discharge planners, social workers, and in-house staff to achieve budgeted census levels. Assists the Mount Leadership Team in outreach activities to enhance the ministry visibility with the greater community.
Responsibilities
- Supports admissions to the facility, in-house transfers and discharge processing for the Skilled Nursing, TCU and Assisted Living programs.
- Works in a team-based setting to work collaboratively with hospitals, skilled nursing facilities, referral agencies, discharge planners, social workers, and in-house staff to achieve budgeted census levels.
- Affords assistance to the Mount Leadership Team in outreach activities to enhance the ministry visibility with the greater community.
Requirements
- 2 years of experience in long term care, clinical case management and admissions.
- 2 years of experience in a customer service, business development or sales environment.
Qualifications
Bachelor’s Degree in Business, Marketing, or Human Services is preferred.
Skills
Strong communication skills, ability to work in a team-based environment, and proficiency in Microsoft Office.

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
