Unity Health and Rehabilitation Center, a 250 bed skilled nursing facility, is seeking a compassionate and experienced Admission Nurse/Supervisor to join our team of dedicated professionals. As a key member of our nursing staff, you will play a vital role in ensuring the seamless transition of patients from the hospital to our rehabilitation center. Responsibilities
- Manage the admission process, ensuring a smooth transition for patients and families
- Supervise and coordinate the activities of the admission staff, providing guidance and direction as needed
- Collaborate with healthcare providers and facility staff to ensure patient-centered care
- Develop and implement effective policies and procedures to ensure compliance with regulations and standards
- Provide ongoing education and training to admission staff to ensure continuous improvement
- Conduct quality assurance and quality control activities to ensure high-quality patient care
