Wraparound Facilitator

About the role

There are moments when a family feels overwhelmed—when appointments, school concerns, treatment plans, and daily challenges seem impossible to navigate alone. As a Wraparound Facilitator, you’ll be the person who helps bring everything together. You’ll sit at kitchen tables, attend team meetings, connect families with resources, celebrate victories both big and small, and help children build the support systems they need to succeed. You’ll work alongside parents, caregivers, schools, providers, and community partners to create a plan that gives kids the best chance to thrive. Responsibilities

• Facilitate Wraparound planning meetings with families and support teams
• Help families navigate the SED/HCBS Waiver and PRTF Diversion Grant programs
• Coordinate services and community resources to support treatment goals
• Develop individualized plans that focus on strengths, needs, and long-term success
• Collaborate with families, educators, providers, and community partners
• Monitor progress and adjust plans to ensure positive outcomes
• Advocate for children and families within schools, agencies, and community systems
• Complete documentation and maintain compliance standards
• Travel within the service area to meet with families and community partners

Pay

$22.36 per hour

Schedule

Full-time, in-person. Typical hours Monday–Friday, 8:00 AM–5:00 PM.

Qualifications

• Experience counts! We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college.
• Strong communication and empathy skills
• Experience working in human services, behavioral health, social services, education, or a related field preferred
• Comfortable in a variety of human service settings
• Able to work effectively with children experiencing Severe Emotional Disturbance (SED) and their families
• Must pass drug screen and required background checks
• Must have a valid driver’s license
