About the role

In support of our mission to continuously improve the health and independence of the individuals we serve, the Wraparound Facilitator provides high-fidelity, family-driven, youth-guided Wraparound facilitation and care coordination services under the supervision of the Clinical Program Manager. The Wraparound Facilitator partners with youth, families, natural supports, providers, schools, and community agencies to identify strengths and needs, facilitate Child and Family Team meetings, develop and monitor individualized Wraparound Plans of care, coordinate services, and support families in achieving their goals. The Wraparound Facilitator promotes collaboration, community integration, crisis prevention and planning, and the development of sustainable natural supports while ensuring services are delivered in accordance with YES Waiver, agency, contractual and state requirements. This position is responsible for coordinating services across multiple systems, monitoring progress toward identified outcomes, advocating for youth and families, and ensuring continuity of care throughout enrollment in the YES Waiver program. The Wraparound Facilitator maintains timely and accurate documentation in the electronic health record, manages multiple priorities and deadlines, and complies with all productivity, quality, and documentation standards. A minimum of 55% of working hours must be dedicated to direct service delivery. Because services are family-driven and youth-guided, work schedules may include evening hours, as needed, to accommodate the needs and availability of youth and families. Successful Wraparound Facilitators are strengths-based, culturally responsive, and committed to empowering youth with serious emotional disturbances and their families to remain safely and successfully in their homes, schools, and communities.

Responsibilities

  • Provide high-fidelity, family-driven, youth-guided Wraparound facilitation and care coordination services
  • Partner with youth, families, natural supports, providers, schools, and community agencies
  • Identify strengths and needs and facilitate Child and Family Team meetings
  • Develop and monitor individualized Wraparound Plans of care
  • Coordinate services and support families in achieving their goals
  • Promote collaboration, community integration, crisis prevention and planning, and sustainable natural supports
  • Ensure services are delivered in accordance with YES Waiver, agency, contractual and state requirements
  • Coordinate services across multiple systems and monitor progress toward identified outcomes
  • Advocate for youth and families and ensure continuity of care throughout enrollment in the YES Waiver program
  • Maintain timely and accurate documentation in the electronic health record
  • Manage multiple priorities and deadlines and comply with productivity, quality, and documentation standards

Qualifications

  • Hold a Bachelors degree in Human Services*; or, if degree is not in Human Services, have completed a minimum of 24 credit hours in a human services-related field
  • Possess a strong understanding of mental health diagnoses and treatment
  • Demonstrate the ability to work both independently and collaboratively in a fast-paced, recovery-oriented environment
  • Proven ability to build meaningful connections with individuals served
  • A valid Texas drivers license is required, along with the ability to maintain approved driver status throughout employment

Preferred Qualifications

  • A Masters degree in Human Services or a related field
  • At least one year of experience providing wellness and recovery support services in mental health, substance use treatment, and/or crisis intervention and screening
  • Actively pursuing clinical licensure in counseling, behavior analysis, social work, family therapy, or a related discipline
  • Preference given to those holding advanced or full licensure
  • Bilingual proficiency in Spanish is highly valued
  • Experience with concurrent (real-time) documentation practices
  • Current licensure as a Licensed Chemical Dependency Counselor (LCDC)

Differential Detail

  • $1.00 per hour – 2 years previous experience
  • Up to $4.00 per hour – BTCS Tenure
  • $0.75 per hour – Bilingual
  • $1.00 per hour – Advanced Licensure
