Wraparound Facilitator

Job Summary

As our Wraparound Facilitator, you will empower individuals and families through coordinated, individualized support. Your responsibilities include developing and implementing care plans, conducting home visits, coordinating the care team, completing assessments, and facilitating meetings.

Responsibilities

• Drive the development and implementation of highly individualized care plans by engaging natural supports, community resources, and professional services.
• Ensure care plans are person-centered, strengths-based, and culturally responsive, aligning with the unique vision and needs of the individual and family.
• Meticulously track progress, solve problems proactively, and collaborate continuously with the team.
• Conduct regular home visits to assess client needs and provide crisis intervention as needed.
• Coordinate the care team including the Child and Family Specialist, Parent Partner, and Clinician to address unmet needs and ensure adherence to the care plan.
• Complete comprehensive assessments as per contract requirements, demonstrating knowledge of psychopathology and comprehensive evaluation techniques.
• Ensure the child's Plan of Care (POC) is provided according to the service plan and continuously reassess the needs of the child and family with the team, modifying the individualized POC as needed.
• Attend and participate in CFT meetings and IEPs to assist in determining a comprehensive plan for the child/children involved in the referral.
• Facilitate the child/family's progress toward goal attainment and monitor child safety as per the service plan.
• Provide referrals, advocacy, support, and accompaniment to various social service agencies as needed.

Requirements

• Required: Bachelor's degree and 1-3 years of experience providing intensive services.
• Preferred: Master's degree in social work or psychology and Wraparound experience/training.
• License: CAP (Clinical Associate Practitioner), psychologist: CAL (California Psychological Association), LMFT/LPC (Licensed Marriage and Family Therapist/Counselor).

Pay Range

$27.98 - $41.62 /hour
