The Wraparound Facilitator plays a vital role in coordinating individualized, family driven services that strengthen natural supports, promote stability, and help youth remain successfully connected to their families and communities. We are seeking a compassionate and dedicated Wraparound Facilitator who is passionate about supporting youth and families through trauma informed, strengths based, and family centered practices. This role is ideal for a professional who excels at relationship building, advocacy, collaboration, and connecting families with meaningful community resources. The Wraparound Facilitator is responsible for leading the wraparound planning process, coordinating multidisciplinary team meetings, developing individualized plans of care, connecting families to community based resources, and monitoring progress toward identified goals. This position works closely with youth, families, schools, behavioral health providers, community partners, and other members of the care team to ensure services are coordinated and responsive to each family's unique strengths and needs. The ideal candidate is compassionate, organized, adaptable, and solutions focused, with a strong commitment to empowering youth and families, strengthening natural supports, and promoting long term stability, independence, and positive outcomes.

Responsibilities

• Receives/responds to incoming calls from referral sources/potential clients and exchanges information to identify the clients' needs and consults with Director of Clinical Management to determine the Company's ability to meet them.
• Completes all viable referrals by setting up and carrying out pre-screening assessments, completion of pre-screening reports, and preparation of case proposals and other related pre-admission paperwork (i.e., obtains authorization for payment, coordinates availability of an appropriate treatment team).
• Oversees, directs and supervises field staff assigned in assisting with pre-screening process.
• Accesses national/state/company account information, including the account names and terms of contracts or other past payer agreements, as appropriate.
