Key Responsibilities
- Assess, treat, and monitor wounds and skin conditions; create and update care plans
- Complete wound documentation within 48 hours and conduct weekly wound rounds
- Track healing progress and adjust treatments based on best practices
- Ensure accurate staging and documentation per NPUAP guidelines
- Collaborate with providers, families, and the interdisciplinary team
- Participate in MDS (Section M) and care planning processes
- Educate and train nursing staff on wound care practices
- Support discharge planning for residents with wound care needs
- Track treatments and support accurate billing
Qualifications
- Active Illinois RN or LPN license
- CPR certification (or obtained within 3 months)
- Wound care certification preferred
- 1+ year wound care experience (long-term care preferred)
- Strong clinical, documentation, and communication skills
- Organized, self-directed, and able to work independently
- Team-oriented with leadership ability and sound judgment
