Wound Care Coordinator (LPN/RN)

Key Responsibilities

  • Assess, treat, and monitor wounds and skin conditions; create and update care plans
  • Complete wound documentation within 48 hours and conduct weekly wound rounds
  • Track healing progress and adjust treatments based on best practices
  • Ensure accurate staging and documentation per NPUAP guidelines
  • Collaborate with providers, families, and the interdisciplinary team
  • Participate in MDS (Section M) and care planning processes
  • Educate and train nursing staff on wound care practices
  • Support discharge planning for residents with wound care needs
  • Track treatments and support accurate billing

Qualifications

  • Active Illinois RN or LPN license
  • CPR certification (or obtained within 3 months)
  • Wound care certification preferred
  • 1+ year wound care experience (long-term care preferred)
  • Strong clinical, documentation, and communication skills
  • Organized, self-directed, and able to work independently
  • Team-oriented with leadership ability and sound judgment
