Wellness Navigator
About the role
The Wellness Navigator at Community Bridges, Inc. (CBI) plays a crucial role in removing personal and environmental obstacles to health care access. This position involves working closely with the primary medical provider, the clinical team, and staff from other agencies involved in the patient's care. The Wellness Navigator is responsible for managing patient care proactively through case management, education, care coordination, systems navigation, advocacy, and support. This role primarily focuses on in-home and community-based services.
Responsibilities
- Proactively manage patient care by providing case management, education, care coordination, systems navigation, advocacy, and support.
- Engage, educate, and offer support to individuals, their families, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up.
- Work closely with the primary medical provider, the clinical team, and staff from other agencies involved in the patient's care.
Requirements
- A high school diploma or GED.
- An associate degree (or higher) in a field related to behavioral health, preferred.
- Education and/or experience in the field of health and nutrition, preferred.
- A current AZ Driver’s License with a clean 39-month MVR.
- To be eligible for the position, you must hold a Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.
- An Arizona Fingerprint Clearance card is required.
Qualifications
