Responsibilities
- Provide health education & outreach activities to individuals in eligibility priority populations
- Collaborate with local community agencies to identify potential underserved participants
- Enroll eligible participants into the CT Early Detection & Prevention Program (CEDPP)
- Assess eligible participants for barriers preventing participation in early detection screening and prevention services
- Complete all required assessments, screenings and documentation
- Work closely with medical providers and clinical personnel to ensure that clients have integrated, comprehensive, coordinated healthcare
- Attend all required program meetings, DPH provider meetings and training sessions
Requirements
- Minimum High School Diploma or equivalent
- BS or BA in Social Services or related field strongly preferred
- 2-3 years of experience in a medical setting preferred
Qualifications
- Bilingual English/Spanish highly preferred
- Excellent verbal and written communication skills a must
- Valid driver’s license and reliable transportation required
- Basic clerical and personal computer skills, including Microsoft Office suite are required
- Flexibility in work hours including some availability evenings and weekends when needed
- Ability to work independently and communicate effectively via phone, video conferencing and in person a must
- Ability to manage multiple priorities and ensure all grant program requirements are followed
