Health Navigator

Responsibilities

• Provide health education & outreach activities to individuals in eligibility priority populations
• Collaborate with local community agencies to identify potential underserved participants
• Enroll eligible participants into the CT Early Detection & Prevention Program (CEDPP)
• Assess eligible participants for barriers preventing participation in early detection screening and prevention services
• Complete all required assessments, screenings and documentation
• Work closely with medical providers and clinical personnel to ensure that clients have integrated, comprehensive, coordinated healthcare
• Attend all required program meetings, DPH provider meetings and training sessions

Requirements

• Minimum High School Diploma or equivalent
• BS or BA in Social Services or related field strongly preferred
• 2-3 years of experience in a medical setting preferred

Qualifications

• Bilingual English/Spanish highly preferred
• Excellent verbal and written communication skills a must
• Valid driver’s license and reliable transportation required
• Basic clerical and personal computer skills, including Microsoft Office suite are required
• Flexibility in work hours including some availability evenings and weekends when needed
• Ability to work independently and communicate effectively via phone, video conferencing and in person a must
• Ability to manage multiple priorities and ensure all grant program requirements are followed
