About the role
The Health Navigator supports individuals by helping them identify available programs and local supports.
Responsibilities
- Establish rapport and offer general guidance.
- Aid people in connecting to options addressing daily needs like food access, transportation, housing-related concerns, and health information.
- Provide practical assistance and serve as a reliable point of contact.
- On any given day, assist with tasks such as explaining a blood pressure reading, locating groceries, arranging transportation for an appointment, or identifying an appropriate program based on their situation.
- Strengthen reach to available resources and help individuals navigate processes that might be challenging to manage.
Requirements
- High school diploma or equivalent required; associate degree preferred.
- Experience assisting individuals from varied backgrounds.
- Knowledge of local programs, resource networks, and available options.
- Strong communication skills and ability to establish rapport.
- Ability to work across multiple processes and assist individuals in understanding next steps.
- Skills in coordination, problem-solving, and identifying available options.
- Ability to maintain accurate documentation and referral tracking.
- Bilingual skills a plus.
