Key Responsibilities
- Coordinate and schedule fitness classes, wellness programs, and health education opportunities for residents.
- Organize guest speakers and educational programs on wellness, healthy aging, and medical topics.
- Plan and coordinate annual flu clinics and other vaccination clinics as needed.
- Welcome and orient new residents to wellness and fitness programs.
- Meet individually with residents to discuss health concerns, wellness goals, and available resources.
- Encourage resident participation in wellness activities that support healthy aging and independence.
- Serve as a liaison between residents, families, and the Coordinated Home Care Program.
- Aid residents and families with home care coordination and referrals to physicians, rehabilitation services, skilled nursing facilities, assisted living, memory care, hospice, and other community resources.
- Collaborate with Maintenance, Dining, Marketing, Activities, and other departments to ensure coordinated resident services.
- Respond to the Emergency Call System while in the building and maintain emergency call documentation and updates.
- Coordinate CPR and AED training for staff and provide orientation to emergency procedures.
- Educate staff on topics related to aging, resident wellness, and healthcare best practices.
- Maintain resident confidentiality and ensure compliance with HIPAA regulations and the organization's Written Information Security Policy (WISP).
Requirements
- A current Massachusetts Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
- Three to five years of nursing experience.
- CPR and AED certification.
- A valid driver's license.
- Experience working with older adults or senior living communities preferred.
- An associate's or bachelor's degree in Nursing, Social Work, Gerontology, or a related field preferred; equivalent nursing experience will be considered.
- Knowledge of community resources, supportive services, and care coordination.
- Strong organizational, communication, advocacy, and problem-solving skills.
- Proficiency with Microsoft Word, Excel, email, and internet applications.
Additional Duties
- Participate in weekly administrative meetings.
- Maintain continuing education requirements to support Massachusetts nursing licensure.
- Support departmental operations and perform other duties as assigned.
