About the Role

In keeping with our organization’s goal of improving the lives of the Residents we serve, the Wound/Treatment Nurse plays a critical role in providing superior customer service and nursing care to all Residents. The Wound Nurse is responsible for performing skin treatments, obtaining treatment orders from attending physicians, and assisting with modifying the treatment regimen in accordance with established policies and procedures. This role provides supervision of staff and safeguards the health, safety, and welfare of all Guests under their care.

Responsibilities

• Daily:
  • Complete a Head to Toe body audit on all new admissions and re-admissions to ensure all open areas are identified.
  • Ensure skin sheets, treatment orders, and preventative measures (e.g., T&P, specialty mattress, heels up orders or boots) are in place.
  • Review residents with new skin alterations and implement skin sheets, treatments, specialty mattresses, updated Braden scale, T&P orders, and care plans.
  • Provide daily education to staff on preventative measures (e.g., T&P, toileting, incontinent care).
  • Hold Nurses and CNAs accountable for preventative maintenance to avoid new wounds.
• Weekly:
  • Update skin sheets every 7 days by a designated close-of-business day.
  • Unit Managers to update care plans weekly for pressure and non-pressure areas by Fridays.
  • Send Wound Report to VP of Clinical Services by Thursday, including the percentage of Center Acquired Pressure Ulcers (goal: 3% or less).
  • Complete treatments for vascular, surgical, and pressure wounds (stage 2 or greater) Monday-Friday.
  • Ensure Charge Nurses treat skin tears and stage 1 wounds, with skin sheets completed weekly.
• Ongoing:
  • Prevent wounds through proactive measures, including daily staff education and rounds.
  • Modify treatments within a 2-week period if wounds worsen, with input from the Wound physician and PCP.
  • Implement preventative items for at-risk residents (e.g., T&P orders, barrier cream with Zinc, pressure-relieving devices).
  • Ensure Braden assessments are completed for all new admissions, re-admissions, quarterly, and with any change in condition.
  • Conduct rounds with Unit Managers, Charge Nurses, and Wound Nurse to ensure proper resident positioning, T&P every 2 hours, heels elevated, and no skin-to-skin contact.
  • Consult with the nursing team on assessment evaluations and assist in planning skin care treatments.
  • Initiate consultations or referrals as needed.
  • Assess residents and records on admission to discriminate between normal and abnormal findings.
  • Act as a Unit Nurse when directed.
  • Supervise C.N.A’s and subordinate staff, ensuring assignments are completed.
  • Administer medications within the scope of practice for R.N. or L.P.N. licensure.
  • Maintain a current report of narcotics received and used.
  • Perform contemporaneous charting and detailed monthly evaluations of each resident.
  • Ensure care plans are followed and assess each resident’s status accordingly.
  • Round with Wound physician weekly and as needed with other physicians.
  • Delegate wound care as needed and complete weekly skin sheets.
  • Maintain infection control and standard precaution practices.
  • Follow safety precautions when performing tasks and using equipment.
  • Maintain resident comfort, privacy, and dignity while promoting a caring environment.
  • Respond to call lights promptly and courteously.
  • Assist with new admissions and re-admissions when directed.
  • Communicate effectively with residents, visitors, families, peers, and supervisors.
  • Attend and participate in departmental meetings and in-services as directed.

Requirements

• Graduate from an accredited nursing program.
• Current State License as a Registered Nurse (R.N.) or Licensed Practical Nurse (L.P.N.).
• C.P.R. Certified.
