Role Overview: The Registered Nurse in Home Health provides comprehensive nursing care to patients in their homes, coordinating with physicians and the healthcare team to develop and implement individualized care plans. Key Responsibilities Develop, implement, and update patient care plans in collaboration with physicians and the interdisciplinary team.Conduct comprehensive nursing assessments, including OASIS and medication reconciliation.Provide high-quality clinical care, supervise home health aides and LPNs/LVNs, and ensure regulatory compliance.Accurately document patient care and communicate effectively with patients, families, and healthcare professionals.Participate in staff orientation, performance improvement initiatives, and patient safety programs.Maintain professionalism, punctuality, and adherence to agency standards, including participation in on-call/weekend rotations. Qualifications & Skills Unrestricted RN licensure in the practice state.Valid driver’s license, vehicle insurance, and access to reliable transportation.Preferred: 1+ years of clinical experience and CPR certification or ability to obtain within 90 days.
