About the role
Provide and coordinate nursing care in patients' homes to ensure quality, continuity, and compliance with regulations.
Responsibilities
- Develop, implement, and update patient plans of care in collaboration with the healthcare team.
- Conduct comprehensive assessments, including OASIS and medication reconciliation.
- Supervise home health aides and LPN/LVNs, ensuring regulatory compliance.
- Document patient care accurately and communicate effectively with physicians, families, and staff.
- Participate in staff orientation, performance improvement, patient safety initiatives, and reduce hospitalizations.
- Maintain professionalism, adhere to standards, and participate in on-call/weekend rotations.
Requirements
- Current and unrestricted RN licensure in the practice state.
- Valid driver’s license, vehicle insurance, and access to reliable transportation.
Qualifications
Preferred: 1+ years of clinical experience and CPR certification or ability to obtain within 90 days.
