Weekend RN

About the role

Provide and coordinate nursing care in patients' homes to ensure quality, continuity, and compliance with regulations.

Responsibilities

• Develop, implement, and update patient plans of care in collaboration with the healthcare team.
• Conduct comprehensive assessments, including OASIS and medication reconciliation.
• Supervise home health aides and LPN/LVNs, ensuring regulatory compliance.
• Document patient care accurately and communicate effectively with physicians, families, and staff.
• Participate in staff orientation, performance improvement, patient safety initiatives, and reduce hospitalizations.
• Maintain professionalism, adhere to standards, and participate in on-call/weekend rotations.

Requirements

• Current and unrestricted RN licensure in the practice state.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.

Qualifications

Preferred: 1+ years of clinical experience and CPR certification or ability to obtain within 90 days.
