About the role
Registered Nurse (RN) in homecare, providing patient-centered care for adult and pediatric clients in their homes.
Responsibilities
- Utilize the nursing process to plan, implement, and evaluate care services.
- Initiate and adjust care plans based on patient needs and physician directives.
- Administer medications and treatments, monitor effectiveness, and document outcomes.
- Respond to changes in patient conditions and communicate with healthcare team.
- Provide education, care coordination, and discharge planning as needed.
Qualifications
- Valid RN license for the practicing state.
- Current health certificate, PPD or chest X-ray, and BLS certification.
- Preferred one year of RN experience.
Key Duties
- Deliver safe, therapeutic, and rehabilitative nursing care.
- Adapt plans as needed.
- Ensure comprehensive patient support at home.
