About the role
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in a homecare setting under the supervision of a Registered Nurse.
Responsibilities
- Delivering and coordinating patient care per physician orders
- Assessing patient condition
- Documenting activities and responses
- Updating care plans
- Recognizing and responding to changes in patient needs
- Educating patients and families
- Adhering to infection control standards
- Providing compassionate care
- Adherence to regulations
- Flexibility to adapt responsibilities as needed
Qualifications
- Active, unrestricted LPN/LVN license
- Current CPR certification
- TB test within last 12 months
- Reliable transportation
- Preferred: six months of clinical experience
- Additional state-specific requirements may apply
