Weekend Private Duty LVN Nurse

About the Role

The Licensed Practical/Vocational Nurse (LPN/LVN) provides skilled nursing care under RN supervision, adhering to care plans and physician orders.

Responsibilities

• Deliver and coordinate quality patient care in compliance with physician directives.
• Assess patient condition continuously, report changes, and document all activities and responses accurately.
• Implement and update nursing care plans, taking appropriate actions to achieve outcomes.
• Recognize and respond to changes in patient needs, ensuring safety and risk reduction.
• Educate patients and families on disease processes, self-care, and prevention strategies.
• Maintain infection control standards and comply with regulatory, institutional, and professional guidelines.

Qualifications

• Graduate from an accredited nursing program with an active, unrestricted license in the practice state.
• Current CPR certification and TB test within the past 12 months.
• Preferred: Six months of clinical experience, home health experience, and proficiency in assessments and documentation.

Requirements

• Physical ability to lift up to 50 pounds, reposition patients, and perform home care tasks.
• Reliable transportation and ability to travel.
• Ability to function in diverse environments with exposure to potential hazards and infectious materials.
• Compliance with health-related requirements, including vaccinations and testing, where applicable.
