Weekend Private Duty LVN Nurse

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes under supervision of a Registered Nurse. Key Responsibilities Include Delivering and coordinating quality patient care in accordance with physician orders.Assessing patient conditions, documenting activities, and updating care plans.Monitoring changes in patient status and implementing appropriate interventions.Educating patients and families on disease management, self-care, and prevention.Maintaining compliance with policies, regulations, and infection control standards.Participating in ongoing education and skill development. Qualifications And Skills Active, unencumbered LPN/LVN license in the practice state.Current CPR certification and TB test within the past 12 months.Preferred: six months of clinical experience and home health background.Strong attention to detail, organization, problem-solving, and communication skills.Physical ability to lift up to 50 pounds, reposition patients, and perform necessary movements.Reliable transportation and ability to adapt to diverse environments. Additional Requirements Include Ability to work independently in various home settings.Adherence to health-related mandates, including vaccinations and testing as required.
